During the Fundo application process, you will be requested to provide some additional documentation to support your application. We ask for this information to verify the details on your application and prevent fraud and have several ways that you can provide these details:
HOW TO PROVIDE DOCUMENTS
1. Directly in your application
The easiest to provide us with the requested documents is to upload directly into your application. Once you have selected your program and signed your agreement. you will be prompted to provide these documents directly into the application via file upload
2. Email
If you experience issues with the upload form, we can also accept the document via email. Note however, since emails must be sorted by our staff, providing documents via email instead of the form upload could delay your processing time.
3. SMS
If neither the upload form nor email work for you, we can also accept the documents via text message. Please call us so that an agent can provide you with the number to send your texts. Note however, since texts must be sorted by our staff, providing documents via text instead of the form upload could delay your processing time.
WHAT DOCUMENTS TO PROVIDE

If your business is registered with the secretary of state and you have been issued an EIN number by the IRS, please also upload a copy of your EIN assignment letter in addition to one of the documents below.
- 1099 independent contractor and you are paid by a company, you will need to provide one of the following:
- your previous year's 1099 Form. This is the form used for filing your taxes. (PREFERRED)
- your most recent paystub,
- Your most recent settlement statement, or
- an invoice that corresponds to a deposit that was received within the last 90 days,
- Self-Employed or a Sole Proprietor and you are paid by various individual people (ei, Freelance, Hairstylist, IT, Handyman, etc)
- If you are paid through 3rd party apps - a Screenshot of your profile
- If you are paid directly by your customers (cash or checks) - Provide a business card, a photo of your branded truck, or any other documentation that will assist us in verifying your business operations.
- Platform Worker (ei. Lyft, Uber, Doordash, Instacart)
- a screenshot of your driver profile showing your name and rating
- Ecommerce Operator (ei. Etsy, eBay, Shopify, Poshmark, etc)
- An account statement from the last 90 days showing your name and the last 90 days of sales activity
